Welcome to the Online Application! Completing and submitting this application is Step 3 of the process. Step 1 is to apply for Care Credit at www.CareCredit.com. Before you begin the application process, please ensure you have the following information READY and AVAILABLE to ensure your application is complete, accurate, and ready for submission. For submission uploads, your documents will need to be saved and uploaded as PDF, WordDoc, or JPG.
Proof of Care Credit Approval or Denial
Please have your Care Credit information ready, including your key number, date of application, and denial or approval information. If approved, we will need to know the amount you have been approved for. This is an application requirement, and you will be unable to complete the application process without this information. If you have not applied yet, visit www.CareCredit.com before proceeding any further. If you are unable to locate the key ID number from Care Credit, please call them to obtain it at 1 (800) 859-9975.
Your Pet′s Medical Information
Have your pet’s medical information ready – including details and costs related to the medical condition you are seeking assistance for, and the names of your general practice veterinarian and specialty veterinarian.
W-2 / Pay Stubs / SSI / Unemployment Info for You & House Mate
Please have this information with you to be uploaded. Information for your significant other or housemate must be included (if applicable). If employed, we will need the most recent W-2 or last 2 paystubs. If unemployed, include your unemployment letter, SSI income, disability benefits, or other proof of income.
Optional: Hardship Letter
A hardship letter is not required, but encouraged. It explains, in your own words, the situation you are encountering with your pet. Be sure to include the current financial situation you are in, and the medical need for your pet. This letter can be uploaded at the end of your application as a word document.
After you have gathered the above information, proceed with the ONLINE application process below. The online application typically takes about 20 minutes to complete if you have the requested information ready and available. This is the safest, fastest, most secure way to apply, and submits directly to our committee. Please remember that a decision can take up to 1 Business WEEK.
If you do not upload financial documents as the application requests, your application will be placed on hold until your information is received. Typically the forms will not allow you to submit your application without uploading these documents. Be sure to have them ready!

Once you have submitted your information, provided we have received all necessary financial documents, etc, please allow up to 1 business WEEK to hear from our committee on a decision.
