Apply
Do your pet’s medical needs meet our mission? Is your pet receiving care from a board certified veterinary specialist? We are here to help! Your application can be submitted here on our website. Before beginning the application process, be sure to read our “About” page first, which outlines our mission and initial medical qualifiers for the Healing Pets Foundation.
Because the application cannot be submitted without an evaluation with the specialist, we do not provide support for the actual initial examination fee. While the application cannot be completed until you have had a consultation with the specialist, we encourage you to be review the application process and prepare the documents as highlighted below. Your application is not considered submitted until we have all of your aspects (on-line form filled out, and most importantly, current proof of income). If approved, we will support costs (other than initial examination) that may have been incurred 1 week prior to your completed application submission and on-going charges. We know it may take several days to receive the information from your veterinary team, so we consider the application “submitted” when we receive steps 1 and 3 from you.
When you’re ready, proceed through the Application Process, beginning with each step, in order. After your application has been submitted, our committee will handle the final steps, review your application, and provide you with a decision. Please allow up to 1 business WEEK after your initial submission for a decision. Please keep in mind that HPF is a partial financial assistance program, and if approved, we can only help with a percentage of your care. You will be responsible to pay the remaining portion of your bill with the specialist according to the terms they work out with you. We are also unable to provide any guarantee of funding, or start of funding. You are financially responsible for all veterinary bills incurred for your pet.
The application for assistance is a 4-Step Process.
-
Step 1: Apply for Care Credit
If you do not currently have Care Credit, Apply for Care Credit from www.CareCredit.com. This is a credit card available to pet owners with special financing and deferred interest options. The application is free to apply, and a credit decision is made immediately. Applying for Care Credit is a necessary and required step. Your application will NOT be processed without providing proof of recent approval or denial with Care Credit. After you apply, please include your approval or denial information with step 3 of the Healing Pets Foundation application process. Though this is a requirement, your acceptance or denial by Care Credit is not a final determining factor for your approval or denial for assistance, and if you feel there is financial hardship not reflected in this financial tool, we encourage you to include an additional statement of financial hardship during the application process.
-
Step 2: Request Veterinarian′s and Specialist′s Assessment
Request Veterinarian′s and Specialist’s Assessment
Before you apply for Healing Pets, please contact your primary care veterinarian and ALL board-certified veterinary specialists involved in the care of your pet to provide their assessments. Please request that both your primary care veterinarian and your specialist(s) submit their assessment forms through our website. These are needed to complete the application process. It is your responsibility to assure that your primary care veterinarian and specialist receive the request for their assessments.
They can access the forms by visiting the “For Vets” tab on our HealingPetsFoundation.org homepage.
You may also share our direct link to veterinary assessments with them: https://healingpetsfoundation.org/vets/
